How the execution of Christa Pike went wrong: An examination

Tennessee prison officials halted the lethal injection of Christa Pike late Wednesday after executioners failed to administer the drugs successfully, leaving the only woman on the state death row alive and hospitalized.
Governor Bill Lee called off all remaining scheduled executions in Tennessee for the remainder of the year and ordered an independent, third-party investigation into the event.
The execution team at Riverbend Maximum Security Institution began administering the lethal protocol Wednesday evening. Witnesses reported that Pike remained conscious and breathing for over an hour after the procedure began. Executioners closed the witness viewing curtains multiple times before prison staff dismissed media observers and transferred Pike to an outside medical facility.
The Tennessee Department of Correction confirmed the protocol did not permit additional procedures beyond what staff attempted, prompting the immediate medical transfer. Lee released a statement declaring that state law requires lethal injections to proceed in an effective and constitutional manner.
Pike received the death sentence in 1996 for the 1995 torture and murder of 19-year-old Colleen Slemmer on the University of Tennessee agricultural campus in Knoxville. Pike was 18 years old at the time of the offense. Her execution would have marked the first execution of a woman in Tennessee in nearly 200 years.
Pike’s defense attorneys spent decades pursuing state and federal appeals. They centered their legal filings on Pike’s age at the time of the homicide, severe childhood trauma, and diagnoses of brain damage and mental illness. Earlier in the week, the U.S. Supreme Court rejected a final petition for a stay of execution, and Governor Lee denied an executive clemency request.
Defense attorneys also raised constitutional challenges to Tennessee’s execution methods in the weeks leading up to the procedure, pointing to prior procedural breakdowns in the state lethal injection system.
